If just your gecko goes in it, you should change the paper towel about once a week.  I clean/disinfect my water bowls every other day, and change the towel every 3rd bowl clean, so that's about every 6 days.

If insects get in it, or it gets soiled in any way, you might want to clean the hide and change the paper more often.
