It depends on the state you live in. I live in CA and these are the requirements for 16-18 yrs old:

To apply for your motorcycle driver license, you must:

Be at least 16 years old

Have had your permit for 6 months

Have completed driver education and driver training or have your California driver license.

Complete a motorcycle rider training course given by the California Highway Patrol (CHP). All persons under 21 are required to complete the course.

Submit the CHP completion certificate (You will not be required to take the motorcycle driving test at DMV.)

For 18+ :

You apply for a motorcycle permit, and then if under 21 you must take the CHP rider course. If over 21 you may either take the CHP rider course (i recommend this as it gives you a discount on insurance) or take the skills test at the DMV.

The skills test at the DMV are the same skills that you will be tested on at the CHP rider course but the rider course there is a lot of practice involved and is pretty much an automatic pass if you made it through the practice.
