How should I document my project management experience for PMP designation?

I have only been a project manager for one year, but I am interested in pursuing this as a long term career and eventually becoming a PMP. I have researched the requirements for this designation, but I'm looking for more details about the work experience.

What is the best way to document my work experience?

Is it sufficient to simply list month by month the projects I have worked on? Will the PMI want references? Should I break this down by hours and process groups?

I am planning on starting a spreadsheet to record this information to avoid trying to "remember" everything I've done 5 years after the fact!

How did you document your experience when you applied? What was the feedback from PMI? Any assistance would be much appreciated.
