How should I format a letter to verify the amount of rent that's paid to me?

You need to be more clear.   Who are you writing?   Since it is paid TO you how could anyone else verify it?

I have to do this every time i apply for a loan.  I just handwrite it on a piece of paper.  THey verify it by looking at my bank statements for deposits.

If you are receiving rental payments this should be under your tax schedule. Outside of that i`m really not sure what your question would be unless you are trying to aquire a loan based on the income(loss) on your rental.
