The job did ask for it. Should I just insert it at the end with the heading 'Salary Requirement'?

  1. Yes I would put that towards the end, not at the end. At least who ever is reading your resume will have to read something about you before maybe rejecting you on the basis of your salary requirements. What they read hopefully will justify them paying you what you want.

If you place it at the end, where they expect to see it, by them looking  at the last page first, your application could be put to one side purely on the amount you require.

  2. Down to Business' answer is perfect.

  3. Usually you don't include that in a resume. If the job posting requires that you include salary requirements, try and add that into the cover letter using a line like this:

"Per your request, an acceptable salary range for this job, based on the description and my research, is $xx,000-$xx,000, not including benefits or supplements. My requirement is flexible and negotiable, depending on such factors as additional benefits, faster salary reviews, and increased advancement opportunities."
