  1. There should already be a ground wired.  It runs from the ground bar in your panel as a large guage bare wire to your incoming water supply pipe.  It is clamped on to the pipe before it goes into your water meter.  If you are on a well, then the ground goes to a copper stake in the ground outside your house.  This is carefully governed by the codes in your province or state.  You should never do it yourself.  It should be done by a licensed electrician, and checked by your city inspector.

  2. Without more information, you're asking for a whole Code Chapter.

    There are minimum wired sizes based on service size.

    What grounds are available;

    Well casing?

    Driven Rod?

    Metallic underground water service?

    Basement re-bar?

    In general, you must connect the neutral and ground busses of your service equipment to as many of these as possible with the proper gauge bare copper wire.

    That is not meant to be taken as a manual.

    There are a lot of details to get right.

  3. there is a ground connection in the box.  there should be a pretty good sized wire connecting the box either directly to the ground (like your yard or somewhere near the foundation)or to another metal object in contact with the ground.
