first u know  where is theoffice of  assistant commissioner of civil supplies for ur residential area. then  u go there  somebody may sell  a form for applying new ration card.  get  and fill up it . paste one recent photo, and enclose one photo along with the application and submit it to the Asst comm. office.

then official will check ur residential area.  then they will issue fresh card to you.

Go to the Food and civil supplies office of your area. You can find the address from the nearest Ration Shop.

Go to the office and give them a photo, address proof and pay some official charges(Misc charges). The government charge is about 50 to 100 bucks. The officials charges would be in addition to it. The more the official charges, the sooner you would get your card. All the best!
