  1. Super glue or double sided tape.

  2. Normally you would use a larger piece, or you could design the assembly so that it is held together by pressed in Teflon pegs, or even Teflon screws etc. If you really want to "glue" teflon there is a possible way. To make bottle cap liners, they can bond Teflon to silicone very well by treating the Teflon with plasma and partially altering a thin layer of the surface, then curing on a layer of silicone rubber. If you took two pieces of teflon, treated as above, and spread a thin layer of heat cure silicone rubber and hot press it it just might work. It might be possible to use other resins for the binder as well. I have not tried it, so I don't know if it works and how strong it would be. Since the final assembly would be limited by the heat & chemical resistance of the binder system used, it might not be suitable for your conditions unless it is designed so the junction(s) between the Teflon pieces are not in contact with with your process fluid, chemicals etc. It seems a rather tedious and expensive operation; I suggest you try to redesign the part so it can be one piece, or parts press-fitted or pinned  together. Also try to only use Teflon where you really need it, because it is kind of an expensive plastic and has these bonding issues.
