    you need to get a form from the Probate Court dept.

    Call your local court that had jurisdiction over your town and ask for Probate, then request they send you a form in the mail to change your name. In my town there is a fee. I don't know if this is the case in your town.

The laws for this vary by state.  You do not say what state you are in.  The fees vary by county.  You do not say what county you are in.  In any event, your best bet is to call your county courthouse for the laws and fees for your area.  This can be done, pretty simple process.  You do not need to get married if you are female.  You CAN change your last name, contrary to what one idiot said.  The main thing is, you DO have to be at least 18.  However, you have posted this in the genealogy forum and it is NOT a genealogy question.  It is a LEGAL question.
