What you need to do is go into account management, then click change gamertag. Normally it costs 800 microsoft points (about $10) but if you have gotten reported Xbox Live should make it free to change your gamertag. So it will be free of charge, if its not appearing call 1-800-4MY-XBOX.

You need to buy Microsoft Points for $20.

At game stores, you get the cards that come with codes that you type in and then once you've signed in (0n live) go to gamertag options > account settings > change gamertag > and it asks for the code.
