If your letter is of particular interest, the appropriate Royal may see it personally, but usually the reply will be drafted by a secretary. If you want a particular department, (say to arrange for a 100th birthday telegram), you can actually phone Buckingham Palace - their number is in the book - but you'll get the switchboard.
