Use Toothpaste (honestly), spread toothpaste over the disc and wipe it in circles with a soft clothe.

Use a Disc Cleaning device, these are usually around 50 to 100 dollars and can be found at many video game stores.

Take the disc to a store that has a disc cleaning machine. Many games stores and rental stores have disc cleaning machines and will cost about 2 dollars per disk to repair.
