How to make a description label for a photography submission to an art show?

Hello. I am submitting a framed photo to my very first art show. For those with experience in galleries/shows, how do I go about making the little card that accompanies my framed photo?

First off, what is this little descriptor card formally called? Second, am I supposed to make/print it? Third, in museums I see that the card is mounted on the wall next to the photo it accompanies... how do I set this up? I don't want to have to tape a handwritten index card to my photo's frame. Are these cards something I can buy somewhere? Finally, what content is typically included on this little card? I would guess the title of my photo and my name... anything else? Thanks for helping a beginner out! :)

The card is mounted on the wall in galleries, but for a contest you should tape it on the back to the back of the lower right corner so that is hangs down and can be read from the front without tape showing.

Include

Title: Photo Name or Unititled #1 - or whatever number you are on)

Artist: Your Name

Medium: 35 mm Photograph (or Digital Photograph...)

Size: (without matt) 8x12 (in inches, width first)

Price: $x*x or NFS (for Not For Sale)

Print it out on cardstock and cut it with a paper cutter so it is nice and straight.  If you don't have a paper cutter or cardstock, go to kinkos.  

P.S. use a boring font like Arial

i call it my little card for my picture; and on it i put:

name i've given image

media (even a photograph i put photograph) and whether it is b&w, sepia, or color (i know ought to be obvious, but some jurers are thoroughness nuts)

whether it is digital or film and what kind of film and what kind of print (injet, silver gel...)

my name  

contact information (phone, email, etc.)

in right lower side i put price
