How to open a HUF account

I am a Hindu , married male . I will like to know the procedure for opening a HUF account.

While applying for the PAN card for the account , what is the date of Birth to be mentioned in the form

for this u need at least 3 member including one male member known as karta

u have to get a pan card of that HUF

DOB will be the date of formation Of HUF
