  2. Well you have to make sure it never happens again but also that it doesn't ruin your life. Did the person get hurt?

  3. Ok, regardless of the setting or your position, you really should:

    1. Tell the patient that you gave the wrong meds. Do not give any other medication, not even the correct meds that you were supposed to give if they have taken the wrong meds already.

    2. Notify the doctor taking care of the patient. If you can't get a hold of the doctor, it never hurts to call the ER or just go there if you can. Call 911 and request for an ambulance if this is too much for you and you are not in a hospital setting.

    Regardless of who you contact, make sure you are able to tell them which medicine you accidentally gave the patient, what medication is he currently taking, and any conditions that you know he has (for example diabetes, angina, high blood pressure, etc.)

    3. Monitor the patient for any changes in consciousness, awareness, breathing, sensations, pulse, blood pressure while you are waiting for instructions from the doctor or emergency services.

    4. Keep your composure. Apologize to the patient. Be prepared to explain what happened and why.

  4. I am sure you know the 5 Rights when it comes to medication.  get in the practice of going over them EVERY time you need to give out a medication.  Even if it is just 325 of tylenol.

  5. QUICK contact the patient and double check that they are ok and change the medication before they die of some weird drug reaction. Also, make sure your medical records are top notch so everyone knows exactly what is happening with that patient, whos responsible for what so they can be contacted, and exactly what treatment they are on!!

    it may not be that severe... but in ur prefession you should always think the worst for every situation.

  7. I had someone dispense a anti coagulant to my Mother and she's a bleeder.

    Lucky she questioned the pill looked different people that do this should know their meds and triple check everything.

  10. What type of facility are you talking about? Hospitals have ways and systems of teaching their nurses how to check medications, the pharmacy in the hospital also has it's own set of checks and rechecks to ensure patient safety.
