How to resolve MySQL problem?

Question

I am having trouble signing in and signing out of my SQL.

When I reset the password it lets me log in but as soon as I click the "Log out" button, it asks me to enter my username and password. I enter the same username and password I used to sign in.

everytime i want to access phpMyAdmin 2.8.2 I have to create a new user and password so it lets me sign in to mysql server.

I think it's not MySQL  problem. It's the PHP thing.
