    I think you're referring to a "care plan". The idea of a care plan is very broad, and is usually tailored to an environment. For example, patients in a hospital have a "patient care plan", students in school have a care plan referred to as an Individualized Education Plan (IEP), and residents in a psychiatric facility have a mental health care plan.

    In addition, the term "learning disability" has a range of meaning: in Europe, this may refer to a profoundly mentally retarded individual as learning disabled (profound MR is an IQ below 20). In the US, a learning disability refers to an individual that has otherwise normal intelligence, but a specific disability with reading, writing, or math (this is simplified).
