How to write an exception letter to the carriers?

I need help writing an exception letter to Blueshield they didn't received my application to add my dependents and I don't even have prove that i fax over those documents because i lost a document they faxed over something when you fax over something to Blue Shield. LET ME KNOW Please I need it by today.....I threw away the document knowing they have process the application to add my dependents no myself.

  1. Dear Blueshield: Please allow an exception to your rule governing the time limit allowed to notify you regarding adding dependent coverage to my plan. I faxed the required document to you on _______; however, this document has obviously been lost because you never received it. Also, I did not keep proof that I faxed it because I assumed my request was being processed.  Blah, blah, blah, etc.

    sign the letter

    make a copy of the letter

    send certified mail

    follow up the letter with a phone call
