  1. It would be easiest if you, or your friend purchased SkypeOut credits.  With SkypeOut credits you can call PC-Phone.

Or, if your friend had SkypeOut credits, you would just call him/her PC-PC.  Then, your friend starts a Conference call to add the PSTN phone call to the 3-way conversation.

Whoever has SkypeOut credits would make the call to PSTN phone line and then add that call to the 3-way conversation using Skype Conferencing features.

For best results, you should be using a headset with built-in microphone.  I use a Plantronics DSP-500 USB headset.  Plantronics makes high quality headsets with microphones.

  2. I am pretty sure you can't, the Skype software will not put through calls without it going through their systems.

Later on this year I will have a voip phone system operating through a broadband connection, similar to Skype but a system that will let you speak to normal land line and mobile phones, in fact it will also allow the use of video phones.

The system is up and running in the USA and all calls over the whole of N. America are free.
