  1. in order to get 0.250 molar solution of Na3PO4,

you weigh out enough Na3PO4.12H2O, to get 0.250 molar solution of Na3PO4

    in 0.500 litres of 0.250 mol / litre = 0.125 moles NaPO4

    so , to get 0.125 moles NaPO4,...

we weighg out 0.125 moles Na3PO4.12H2O:

    0.125 moles @ 380.2 g / mol Na3PO4.12H2O = 47.52 g Na3PO4.12H2O

Your answer: to get what you wanted, weigh out 47.5 grams of Na3PO4.12H2O, dissolve into some water, then dilute up to 500 ml
