  2. Not difficult. However the drain will have to be below the shower at its lowest point in the floor. If the sink drain goes through the floor it will be simple. The water lines may be ran anyway because the water is under pressure and will go up.

  3. Well you can do anything you want, however what matters is how much your willing to spend. as long as the drain lines go to the floor and you have space in the flooring to install a drain, it shouldnt be to hard for a plumber. but the drain is the hardest to figure out

Like fireman says - the hard part is the drain.  Most sink drains go through the wall, but then the drain might head down to the floor.  By tearing out the wall at that point you might be able to get to the drain at ground or below ground level.  If you have a crawl space under your house, the project becomes much easier.  If you do all the work yourself, the expensive parts are the shower surround and fixtures.
