  2. If you plan to go to the US or any other country for that matter to look for a job - get a work visa. NOT tourist ones. You will not be allowed to work in the US if you are on a tourist visa, and no hospital will take you in- that is just illegal and you could get jailed and deported and forever barred in coming to the US.

  3. Well, since your chances of getting a US tourist visa are zero without proving you are employed and have a job in the Philippines to come back to I'd say you need to put it down.

    Here's some advice for you when dealing with the United States embassy, tell the truth.  I lived in the Philippines 7 years, so I know how things are done there, but in the United States the truth is always your best bet.
