I'm applying for a scholarship and the ask for a personal history?

I'm applying for a scholarship (moon lake electric) and they say i must include a personal date narrative not exceeding two pages, double spaced. It should include:

1. a personal history

2.a description of the student's involvement in local club, school, and community activities

3. a description of the student's career goals, and how the student intends to achive those goals

4. an explanation of why the student feel he/she should be awarded the assistance

I'm getting so flustered with all of this...i don't even know how to go about writing this! also, i don't know what they mean by 'a personal history'. if you could answer these questions or have any ideas for me, they would be much appreciated!

try to mesh all of these objectives into a coherent story.  i've written a short example.

i've performed ballet since i was six (objective 1) and once i was older, have enjoyed performing at the community center and volunteering to teach children the activity i love so much (objective 2). since i enjoy this so much, i hope to study dance at ______ university in order to open my own studio (objective 3). with the assistance of your scholarship i will be able to fulfill my lifelong dream of being a professional dance instructor and teach others about the activity i love so much (objective 4)

of course you'll want yours to be longer and more descriptive but i think that format will be the easiest and most interesting to read.
