  1. First make sure with your school district that you even need a letter.   Download the forms you need and fill them out.  They should have very exact instructions to follow to get it approved.  Get a letter from your Dr. or hospital verifying the birth (or whatever they say.)  Write a friendly semi-formal letter to your principal (or fill out school form if any) like:

    I am writing to let you know that I am applying for a maternity/parental leave from teaching for the school year August, 2008 to June, 2009.  I hope you will hold my job for me for the following school year, and I look forward to rejoining the faculty at x*x school in August, 2009.
