She's been with her boyfriend for 3 years. She says after they have s*x she smells like fish. She doesn't stop smelling like fish until she goes to the doctor for pills. She says when they argue and don't have s*x for months she smells fine again. Why is this? They both are known for cheating in the past.

Her GP needs to do some tests as she could have some kind of infection inside which may flare up after s*x.  I know that often after s*x, if the man has ejaculated inside and the woman hasn't showered (as you often don't get out of bed to have a shower unless you have OCD after s*x at night) that causes a 'fishy' smell but it usually goes after a shower.

Try and persuade your 'friend' to go and ask for proper tests and not give up till she's completely satisfied it's not medical.

Sounds like her boyfriend has an infection and keeps passing it to her. He might want to get it checked out too.
