Write details in a notebook if you want, but I recommend you record summaries of the pertinent daily activity on a month-long calender.The visual impact of blank days when you didn't study or exercise is motivating, and the summaries help you determine patterns, particularly if you transfer data to a graph.

    Modifying your behavior has a strong affect on your attitudes.   Force yourself to smile, and you feel less depressed; make yourself go to parties and you feel more socially-oriented.  You may need to seek out a new circle of friends and social venues, as the ones you associate with now were selected based on attitudes and habits you wish to change.

    Consider long-term goals and short-term steps to reach those goals.
