I'm looking to type and print a UB04 form directly from my computer.?

The National Uniform Billing Committee (NUBC), UB04, I have the form, but can't type on it. Does anyone know where I can get a copy that is interactive and I can type onto? http://www.ub04.net/downloads/UB04_10-23-06_B%20W%20PDF.pdf

  1. In order to type on (fill in) a PDF document, it must have been created using a "rights-enabled PDF format". Only the rights-enabled format will let users comment on, fill, save, distribute, submit, and add digital signatures to the form. When you open a rights-enabled PDF form in the free Adobe Reader, a dialog box appears outlining the extended capabilities of the PDF file.

    I searched several websites that talk about and explain the UB04 form but none were in "rights-enabled PDF format".  I suspect the various companies dealing with the UB04 want people to have to buy the forms from them or use their paid services to fill out the forms. One possible work-around to this problem might be to take a screen shot of the form, paste it into a photo editing program (like Microsoft Paint) and use the Text tool to fill in the numerous fields. Save the finished form as a JPG/JPEG image in MY Pictures.  It can then be scanned, printed, or sent in an email as a photo attachment.
