What form I need to fill out from the NY Dept of Labor website if my former employer has not paid me for several months?

It is all just so confusing to me..

If you visit www.dol.gov you can find the address of your local DoL office.  Pay them a visit and explain your situation.  They can give you the correct form to complete and you'll get a contact to deal with in future.

You need to file a claim for unpaid wages with your local office of wage and labor. They will follow up with your employer on your behalf.

Why not call them, ask for enforcement/wage and hour and ask  them which is the correct form.  They probably have an 800 number and you will get better advice.
