  1. ditto - look at www.irs.gov, you can down load the form.


  2. You can order free transcripts from the IRS, either by phone or by submitting Form 4506-T.  Transcripts take about 10 days by mail or can be faxed in a few minutes if you have access to a fax machine.

    If you need copies of the actual returns as filed, plus copies of any attachments, there is a $39 charge per return.  Submit Form 4506 to request those.  These take about 6 weeks.

  3. you can call the IRS to get a copy of your transcript.

  4. See http://www.irs.gov/taxtopics/tc156.html - a transcript should give you the info, just in a different format (computer printed) than an actual copy.
