I have never smoked this much and it is starting to bother me sooo bad . Physically as well as mentally. Because of med I take the patch is a no-go. Any body been there? Please help. PLEASE

there are other alternatives to the patch when it comes to nicotine replacement therapy.
Although I have never tried any myself.  The inhaltor and gum are by far your best options.
However, you may find yourself relapsing back to cigarettes depending on your nicotine dependancy.
Cutting down the amount you smoke, though difficult helps you quit then switcing to the inhaltor or the gum may help you quit altogether.
Though for you cold turkey could work a treat, and although it is difficult for the first week gives you instant results.
