I Was Born In India But Raised In The UK and I Have Lost My Birth Certificate??

I was born in New Delhi but raised in London UK...My birth certificate was an Indian birth certificate...How Can I obtain a new one????

Another thing is that I have changed my name slightly...So I am confused as to how I can move forward and obtain a new birth certificate but one that reveals my new name....
