  1. Call the Labor Board or look it up online.  Possibly the unemployment office in your area.


  2. Your school should have them.  Ask in the guidance office or main office.  I think I remember doing this in high school.  Good luck.

  3. You should be able to get them at school. Thats where I got mine. I went to the guidance counselor and they tell you to get a physical (go to dr.) and then they fill it out for you. The process takes like 2 days depending on when you get your physical.

  4. You can try at your schools guidance counsellings office, they usually have papers available

  5. well, usually you can apply online on the website or you can go into the workplace and ask the manager for a form, but hate to break it to ya, but depending on the place or area you live in, people might not take people under 16. I'm 15 and I was looking for a job, but the minimum age is 16
