Call the Labor Board or look it up online.  Possibly the unemployment office in your area.

Your school should have them.  Ask in the guidance office or main office.  I think I remember doing this in high school.  Good luck.

You should be able to get them at school. Thats where I got mine. I went to the guidance counselor and they tell you to get a physical (go to dr.) and then they fill it out for you. The process takes like 2 days depending on when you get your physical.

well, usually you can apply online on the website or you can go into the workplace and ask the manager for a form, but hate to break it to ya, but depending on the place or area you live in, people might not take people under 16. I'm 15 and I was looking for a job, but the minimum age is 16
