1. This depends very much on the type of plan that you have. If it is an HMO, call MAY have to call your primary care physician's office. You will then have to go in for a referral for an evaluation visit with a "Mental & Nervous" provider.

If you have a PPO, (or even an HMO) there is probably a separate vendor for counseling. Call their regular member services number and they will refer you to a single name in your area that you would call to set an appointment.

Usually, only one to 3 visits are covered, to evaluate and stabilize the patient. Then the BC/BS will determine if things are bad enough to provide more visits. Usually, you have to fall into one of a handful of severe mental categories, be unable to work or function, or be at risk of hurting yourself in order to qualify for many more visits under most plans. Also a deductile and benefit maximum may apply.

I find the plans are good for a couple of visits to pull things together, and then you can decide if you want to continue with that or another therapist at your own cost.

  2. Depends on 3 factors.

Is your employer a large enough employer to design their own benefits? If so, your company may have either eliminated or enhanced what was the default. Ask your HR person for a copy of your benefits where it would state this.

You could call the number on the back of your id card if there.

You could just call around, find a counselor who is in the BCBS network, then ask them - the counselor probaby has a computer program where they can get your coverage information from BCBS.

  3. Each plan is different, even within BC/BS Michigan. Your best bet is go to http://www.bcbsm.com then click on "I am a member" and follow the link to log into your account. You should be able to view your benefit guide for your plan.

If you can't find the information you are looking for there, you can contact your HR benefits administrator or BC/BS customer service at 1-800-637-2227.
