I am doing a training course for a class and need to ask for feedback?

I have to write a feedback sheet with little boxes and questions. What are some good questions to ask the students?? I have never done a training course before! Help me please!!

Well, I think that definately depends on the content of the class, but the typical stuff would be:

When did you feel most engaged in the class?

When did you feel least engaged in the class?

Was there something from class that you found particularly helpful/surprising?

What were your goals in coming to class? Have those changed? Have those goals been met?
