I am filling out a "not for public record transfer tax form" and I need help! Which do I choose?

It says:

The amount of tax due on the accompanying document is _________

and there is also

______ Computed on full vale of property conveyed

______ or computed on fullvalue less liens and encumbrances remaining at the time of sale

Not sure how to fill it out? There is no loan on the property and the tax I calculated is 210.22..

Help! What do I fill out?

  1. If the accompanying document is the transfer return itself, put 210.22 in the first blank and leave the other two blank.  

    If this segment is part of the actual transfer form, then leave the first blank empty and put the 210.22 in the second blank and leave the third empty.

    In the second case, be sure the accompanying form, if any, doesn't require an additional tax payment.   My guess is you don't have another document, in which case you just use the second blank
