I am leaving the country with a friend and her dad?

Question

what documents do i need? i am a minor so i know i need a notarized paper from my mom giving him consent to take me out of the country and a seperate one giving him consent to give medical attention when needed, but what do they need to say on it?

i know it sort of sounds like i'm answering my own question, but i really have no idea what needs to be on there.

and aside from those two documents is there anything else that i will need?

Youll need a passport to prove your citizenship and eventually a visa. weather or not you need a visa depends on what country youre going to. Remember that youre dad need the same documentation as well. Johan
