Don't know if this is what you're looking for, but this what we guys follow for a lineup of 6 players a side (3 levels):

N - Forward Setter and Blocker

L - Left Spiker and Blocker

R - Right Spiker and Blocker

C - Centre

P - Back Passer

S - Server or Back Passer

Usually L, C, R are along the same line & the three horizontal rows are equispaced. But occasionally C drops a little back, in-between the two back levels, depending on how good the opponent server is. When the L or R goes towards the net to block the ball off an opponent's spike, the formation has to adapt/change in such a way that there is not much open space given to the opposition to spike the ball into. After that, we revert back to the original setup immediately.
