  1. Go to google and put in Communication Questionnaire Templates. Review and use them for a reference to compose one of your own.

    You can also go to a library and/or your school and look at others that have done them and compare them to what you have done. Have others review yours and ask for feedback.

  2. I not sure exactly what you looking for. I know sometimes with this age group I shoot over the head, but, when I see the confusion in their eyes I know to change my teaching approach.

    1. What do words mean to you?

    2. How are words used to explain something?

    3. How can you know a person not understand what you say?

    This is the questioning approach I would try and adjust for their level of understanding the question. These questions may be a little deep for pre-school.

  3. You could have a questionnaire  which is read to the child by an adult then the child would fill in a blank face with a smile they like or a sad face for dislike. Communication is in all area of a child's development weather the child is listening talking or taking part in social activities. Children will communicate with you if its fun for them. Good luck
