  1. If you go to a gourmet cooking store, you will find Krups, Cuisinart, several Italian companies. My expirience, with Krups is good, Pavoni has been wonderful. The newer models now are a bit safer to use, but don't heat the water, or milk as well for steam. Go to Gaggia, that is also a great one. Its what many coffee houses use.

  2. The biggest piece of advice I would have is to buy a pump-driven machine as opposed to a steam-driven machine.  The problem with the steam-driven ones is the you have to wait until enough steam builds up to pour your shots and steam your milk but often times you will run out of steam and your milk won't be hot enough.  And most of the steam-driven machines require you to pour your shots and steam your milk simultaneously.

  3. Well, that very much depends on your budget

In my experience low priced espresso machines are a disappointment

The first step in any case is to buy a grinder (the number of people who buy an espresso machine, yet continue to buy ground coffee astounds me)

If you want to upgrade your coffee experience considerably, but do not wish to make the considerable investment that an espresso machines demands, buy yourself a basic burr grinder, get some fresh whole beans from your local gourmet coffee roaster, and a Swissgold filter for around USD12.  Sure it wont be espresso, but you'll be amazed at the taste revolution.

A cheap espresso machine will not deliver an espresso in the sense that you are familiar with at your local high end cafe.

I would suggest machines less than USD500 are probably going to disappoint you, although obviously this is difficult to gauge as I know nothing of your expectations
