If you are staying in Kammanahalli, please go to the Kammanahalli Revenue Office ( Kammanahalli Panchayat Office ). Take an address proof and your age proof ( SSLC or 10 th standard marks card ). The concerned clerk, at the office will give a simple form. Fill it in front of him. Ask clearly your ward number and fill the same, enclosing the photocopies you have taken.
