I am taking paracetamol and ibuprofen for extreme tooth pain but can i take codeine phosphate as weell?

You should be okay taking some codeine as long as it doesn't contain more paracetamol or ibuprofen.  It probably will because pure codeine tablets are few and far between these days.  Otherwise, stop taking the other meds, wait about four hours and then start the codeine tabs.  If you have further concerns, I would recommend ringing a pharmacist.

You shouldn't be mixing drugs.  Either take the ibuprofen or codeine.  The ibuprofen is usually the most effective for tooth aches.  Good luck.
