Model: Nokia 2610. I have been with ATT for less than 3 months and hence would not give me the unlock code.

Is it also true that after repeated unsuccesful attempts at unlocking the phone, the handset gets permanently locked?

Yes after multiple attempts your phone will be permanently locked and you will have to go to the store.
