I amleaving the country....?

i am leaving the country for 9 days.... i have a four month old daughter and i need to write a letter stating that my parents get full parental responsibility for my daughter while i am gone.. i have no idea what to write.. can any please note the things that i should include in my letter... please only serious answers.. thanks !

When you write a letter for medical care it is good for 6 months. All you have to do is write "I give _____________ and/or ______________ permission in my absence to make and sign for any medical care my daughter ____________ (put her name in that blank)." Sign and date it and give them a copy of her medical card. I had to do this when my son stayed with my parents when my husband and I were out of town for the weekend.

Make sure you write on there that they are able to take her to the doctor and make any medical decisions. Give them her insurance card, too just in case.
