  1. It takes a little bit of experience working with patterns. get an older person to help if you are new at t.

Buy a puffy sleeve shirt pattern in the closest size you can. One size larger would be best if you can't get your exact size.

Pull a brown paper bag apart so that you get one big piece of brown paper. Put your short sleeve pattern on it and secure with a few pieces of tape. Now put the long sleeve pattern down over the short sleeve, matching the points where the underarm seam will be. Fold the bottom of the long sleeve up and out of the way.

With a pencil, trace the top of the long sleeve pattern from the shoulder  down as the underarm points. The rest of the way, trace the short sleeve pattern. un-tape both pattern pieces and make any lines that you need, to turn it into one smooth sleeve pattern piece.

To construct the sleeve, use the instructions from the short sleeve pattern envelope. To attach the sleeve to the shirt, use the instructions from the long sleeve pattern envelope.

  2. Take your pattern and trace off the sleeve pattern you have onto a large sheet of paper.  Remember to include all the markings from the original pattern.

Cut the copy to a couple of inches longer than you actually want the finished sleeve...

Cut the copy pattern down the center.

Do the same about 2" either side of this cut, so you have 4 bits of pattern...

On a second larger bit of paper, spread the pieces of pattern about an inch to an inch and a half apart, depending how much gather you want: glue them down to another sheet of paper.

Use a pencil to join up the gaps in the pattern and cut out: You now have a pattern for a puffed sleeve.

To make the gathers, sew a row of gathering stitches across the bottom, just inside the seam allowance, and leaving about an inch at either end to allow for seam allowances...  Sew another row of gathering stitches between the two notches on the upper curved edge.

Make up the sleeve and fit in into the armscye of the blouse (the arm hole), gathering the curved edge to fit.  Gather the bottom edge to fit the arm, with an inch or so of ease for comfort and movement.  Finish the bottom edge with some self bias binding cut from the fabric left over from the blouse.
