Its scripted. Like movies,tv shows etc. But the risks ARE real and sometime injuries do happen by accident. Also sometime they are busted open for real and require stitches.

It isn't real, but it isn't fake.

It is "scripted". A lot of the risks they take amongst many other things are real though. Do not be fooled, what they do requires great athletic ability. I'd like to see just any guy walk in off the street and do a whisper in the wind or jump on the turnbuckle and do a perfect shooting star press.
