  1. Why not do one of the ballys programs?  Get on the ballys fitness page and read the programs and about quick sets.  You can do those which will build muscle but also speed your metabolism.  Do that one day and your cardio the next and keep alternating like that.

    You don't necessarily have to lift and run all in the same day.

  3. As I understand it, the larger your muscles are, the greater the strain on your heart, therefore the stronger it needs to be for you to keep healthy. For myself, however, this is like asking how many plasters you need to maintain your habit of hitting yourself with a hammer. For the most effective and most wildly varied physical exercises of all kinds, I would look for a local kung fu master.
