I found out that my relative died in 1961 in a Newyork city Hospital that has closed

But i can not find a death certificate in the NYC death index for him.The name of the Hospital was Stuyvesant Polyclinic hospital or called Polyclinic hospital.How can i get his death certificate?

The death index isn't completely transcribed and doesn't include most of the Brooklyn records. So contact the Brooklyn Health Dept for a copy of the death certificate. If they really don't have it, they'll tell you. Then you contact the State of NY for a copy. I'm betting Brooklyn has it, but you're looking in Manhattan records with the index.
