  1. When people get a blow out fracture of the orbit, the fracture can entrap the inferior rectus muscle which pulls the eye downward when it's looking to the side. If that muscle and nerve combination were damaged, and they released those tissues at the time of surgery, you should slowly regain the function of the muscle with time. It may take some time depending on how severe the damage was to the muscle itself or to the nerves.

If tissues are still entrapped, you'll continue with the diplopia until someone releases that traction. You might also have an injury that after the release, healed with a scar that is holding the muscle and lower tissues within it. I'm not sure if an MRI would show this, but it would be one way to 'see' if there's continued tissue damage, or a fibrous scar or fibroma that's still entrapping those tissues.
