  1. Replacement....  but be sure to send in a photo and all your new information.

    It would probably be a lot easier to just apply for a new one.

    You could still apply for a new one.

    You will need your Birth certificate and someone to confirm your ID .

    Ask your GP to sign

  3. You'll need to apply for a new one.  I just went through the same thing.  Your passport is good for 10 years, after which you can renew it, or it will expire.  If it expires, you have to go through the application process all over again.

  4. If you visit the website it will guide you.  I believe that since your old passport expired long ago you will be required to order a new one.  The process is rather straight forward and you will have to go to a local post office to submit your application.  The application can be obtained on line.  Hope this helps.
