I have a NHS HC2 certificate - how do i get the optical voucher itself??

I need the voucher itself asap, do i have to send off for it , or get it in an opticians?

after you get your eyes tested they fill in the forms for your free specs

you have to fill it out yourself, then once it comes back it'll tell you if get any thing and they will send you your voucher at the same time.

it should be explained somewhere in the forms
